Below are descriptions of the different lifestyle and care options available and what they mean here. No matter where you start at Saint Therese, we’ll evolve with you through our continuum of care.

Transitional
Care

  • Short-term stay post hospitalization providing 24-hour skilled nursing care with emphasis on accelerated recovery
  • Covered by most insurances including Medicare; a qualifying hospital stay may be required
  • Combination of rest and rehabilitation therapies
  • Available at Robbinsdale, New Hope, Brooklyn Park and Woodbury

Senior
Apartments

  • Apartment style living, as social or private as you prefer
  • Month-to-month lease agreements
  • Private pay; elderly waiver accepted at New Hope after one year of private pay
  • Includes: some meals, regular Mass and ecumenical services, activities, transportation for scheduled outings, spiritual support and wellness membership
  • No health care services included however, residents are welcome to receive care from Saint Therese home health services or other health care partners
  • Available at New Hope, Brooklyn Park and Woodbury

Assisted
Living

  • Apartment style living with required care packages based on needs; some packages tailor to memory care
  • Month-to-month lease agreements
  • Private pay; elderly waiver accepted at New Hope after one year of private pay
  • Includes: meals, weekly housekeeping, regular Mass and ecumenical services, activities, transportation for scheduled outings, spiritual support and wellness membership
  • Available at New Hope, Brooklyn Park and Woodbury

Long-term
Care

  • 24-hour skilled nursing care; shared or private rooms
  • Daily service rate based on level of care required
  • Payment options include private pay, medical assistance (Medicaid), VA benefits and long-term care insurance; varies by location
  • Assistance with daily living activities (such as meal preparation, dressing, and medication management)
  • Coordination and support with transportation and errands
  • Includes: meals, regular Mass and ecumenical services, activities, transportation for scheduled outings, spiritual support and wellness membership
  • Available at New Hope, Brooklyn Park and Woodbury

Memory
Care

  • 24-hour skilled nursing care; shared or private rooms
  • Payment options include private pay, medical assistance (Medicaid), VA benefits and long-term care insurance; varies by location
  • Assistance with daily living activities (such as meal preparation, dressing, and medication management)
  • Specialized memory care activities and cognitive support
  • Includes: meals, weekly housekeeping, regular Mass and ecumenical services, activities, events, spiritual support, and wellness membership
  • Available at New Hope, Brooklyn Park and Woodbury

Hospice
Care

  • 24-hour skilled nursing care; emphasis on comfort over curative efforts; also known as palliative care
  • Payment options include private pay, medical assistance (Medicaid), VA benefits and long-term care insurance; varies by location
  • Assistance with daily living activities (such as meal preparation, dressing, and medication management)
  • Independent hospice provider selected by family offering additional care at Saint Therese location
  • Available at New Hope, Brooklyn Park, Shoreview and Woodbury
